A Little Help Along the Way
Caring for an aging parent often means navigating unfamiliar territory. Finding reliable services, tracking medications, understanding medical decisions, and knowing which documents you need can quickly feel overwhelming.
At Your Elder Ally, we're developing a collection of practical tools and trusted resources to make these responsibilities a little easier. These resources are not yet available, but we're working to make them useful and easy to access. Our goal is simple: help you find the information you need, stay organized, and make informed decisions about your parent's care, whether you live nearby or hundreds of miles away.

We're working on the following resources for families caring for aging parents:
Pinellas County Resource Directory
A searchable directory of local services and organizations that support older adults and their families. Find help with in-home care, transportation, meal services, dementia support, senior living options, and more.
Medication Organizer
A practical tool to help families organize medications, track dosages and schedules, and maintain a clear medication list to share with caregivers and healthcare providers.
Florida Healthcare Documents & Forms
A collection of important forms, with plain-language explanations to help you understand what each document does, why it matters, and when you might need it.
HIPAA Authorization Form: Allows your parent to authorize healthcare providers to share specified medical information with you or another designated person.
Designation of Health Care Surrogate (Medical Power of Attorney): Identifies someone your parent trusts to make healthcare decisions on their behalf, according to the authority granted in the document.
Living Will: Records your parent's wishes regarding certain medical treatments and end-of-life care if they can no longer communicate those wishes.
Do Not Resuscitate Order (Florida Form DH 1896): A medical order for individuals who do not wish to receive resuscitation if their heart or breathing stops. This form has specific Florida requirements and must be completed with an authorized healthcare professional.
Authorized Representative Forms: Forms that may allow you to communicate with Medicare, insurance companies, or other agencies about coverage, claims, benefits, and related matters on your parent's behalf.
